Adoption Application

Thank you for your interest in adopting a senior dog! To help us match you with a senior dog that is the best fit for you and your family, please complete the entire form below. Incomplete applications will not be considered.

1. Only submit an application for a currently available dog. We do not save applications or keep them “on hold”.
2. Before completing an application, please read through and understand our Adoption Policies. If your application does not comply with our Adoption Policies, it will not be considered.
3. Please allow up to 10 business days for us to process your application. Once your application has been processed, a CSDRH representative will contact you via email or phone.
